ISSA - International Slalom Skateboarding Association

Does anyone have a copy of a waiver signed by skaters at events? I am trying to get the final approval for an event and the town is looking to see what kind of liability that they might be faced with. I am also in need of a copy of the insurance policy used to cover the venue ect.. If anyone could post a pdf with this info it would really help our situation. The Jersey crew is trying to pull off a fun event in the NY Catskill Mts. and the final vote will be held on March 5th. Stayed tuned and thanks for your help.
